No Marks or No Response by Due Date?
The “Follow Up Transactions” document explains how and why to submit a follow-up transaction if one or more facility owner(s)/operator(s) fails to respond in any way by the due date or if you did not agree to, or don’t understand, the response you received.

Landlord Notification of Excavation Work Form
Arizona 811 provides excavators who will be digging at registered apartment or mobile home community with a Landlord Notification of Excavation Work Form that is already completed. Excavators can view the form here and/or download it, complete it themselves and send it, by certified mail, to landlords that are not registered with Arizona 811. Landlords who receive this completed form are required to respond to the excavator within 10 working days, excluding weekends and state-recognized holidays.

Landlord Registration Form
Landlords: State law requires you to register with the one-call notification center. In Arizona, it is Arizona 811. Complete this Landlord Registration Form for mobile home and apartment communities and return it to Arizona 811 by email or fax.
